Birth Certificates

Mail-In Application for Copy of Birth Certificate (PDF)
Please include a self-addressed envelope with your application to receive the Birth Certificate back to you.  

Death Certificates

Handicap Parking Permit (Village Residents Only)

A signed physician's note stating the disability along with driver's license must be provided to the Registrar's Office. There is no fee for this service. 

4x4 Permit (Village of Southampton) - In Person 

You may visit Village Hall at 23 Main Street, Southampton, NY during the hours Monday-Friday 9am-4pm with the documents listed below to receive your 4x4 Permit.

4x4 Permit (Village of Southampton) - Mail-in Directions 

You may apply for the 4x4 Permit by submitting the following by mail to:
 23 Main Street, Southampton, NY  11968

Documents required:
1. Copy of the Receipt for your 4x4 PERMIT issued by the Southampton Town Trustees
2. Copy of either the VEHICLE REGISTRATION or the Town Trustee 4x4 PERMIT
3.  Payment as follows: 

1-Year Permit: $25.00 
3-Year Permit: $60.00
1-Year Senior Permit: $10.00
3-Year Senior Permit: $30.00
Replacement Permit: $10.00

4. SASE - Self-addressed, stamped envelope. 
Please note that permits are processed as received and are mailed upon completion.

Tennis and Pickleball Permit (Village Residents Only)

$75.00 /family permit or $25.00/ individual. 
For court access, there is a $150.00 refundable deposit for the Key Fob* (Credit card only)
* Key Fobs must be returned by December 31st to receive your refund
